What gearing are they using in the mountains?overtrained

Do Lance & co use a regular 53-39 and a 11-23 cogset in the mtn stages? or do they go smaller?

for the majority of riders. For some of the real nasty climbs they might put something smaller than a 39/23 and the press/riders, etc. always make a big deal out of it. If anything, the good climbers, strong riders etc. use bigger gears than a 39/23, sometimes running a 42 on the front and maybe only a 21 on the back.

I read that Lance uses a 53/39 and 11-23 almost exclusively on the road bike for races.

Keep in mind that he's climbing at a low of about 15 mph, which is 112 rpms in a 39x23.

Lance often builds custom cogsets for certain stages. When he won L'Alpe d'Huez, I read he used a 39x24. The next day, when he won the MTT at Chamrousse, he used a cogset that had 21-22-23. Tyler Hamilton also followed Lance's lead that year, so I suspect he does the same thing now.
