I just started with these problems recently and have not changed a thing on the pedals or cleats all year. I think I may be developing a Cummulative Trauma disorder or something in my right ankle/heel area.

I am assuming that I have the cleat set up wrong for me. I have Look 357 pedals with the red cleat. The pain is on the inside of my right heel and I really don't notice it much while riding but it really sets in after the ride to later in the evening.

Any suggestions?

so this may not be useful for you, but I find the fixed cleats leave my heels almost pain free. I've used the red ones when they came with the pedals, but only buy the black ones cause they work so much better for me. I also find that doing toe presses at the end of rides is a big help. No weight, just slowly letting my heel drop below the bottom step and quickly pressing up as high as I can on my toe.

You might consider a pedal with a larger platform, like Time Impacts, also make sure you have enough lateral movement. I was using a shimano style pedal and developed foot pain on the ball of my foot; swithched to Time and no problem since.
