I have fairly regular pain in the outside of my right foot when I ride. I've tried many of the suggestions posted on the board but none have worked. My problem, in my opinion, is that my right leg is slightly longer than my left. My foot wants to roll-over in my shoe but is prevented by being locked in. There isn't enough lateral adjustment in my cleat to compensate. Anyone else have a similar issue?

putting in a footbed in one shoe to take up some space on just that side? If you suspect a slightly longer on one. Then bump up the seatheight a tad to see if this keeps the foot from rolling over? just a thought. Have you tried riding for a bit without clipless to see if you still have the pain when not locked in?

in order of increasing expense:

off-the-shelf orthotic insoles, e.g superfeet, $20-30
heat-moldable orthotic insoles, $40-50
custom podiatrist-molded orthotics, $150-200
Look CX-7 pedals w/ angle adjustment, around $175 (and I would see a podiatrist before using these too)

If your problem is indeed leg length, using speedplay pedals allows shims to be installed between cleat and shoe for correction.

Similar problem here. I "cured" it by loosing my shoe at mid-foot. For me it is espically painful immediately after the ride, also my left leg is shorter, but symptom free!Go figure.
