|rear derrailer problems - help!||ballbite|

|I have an Ultegra rear derrailer with about 6000 miles on it. I noticed about a month ago some hesitation on shifting - nothing too serious. Someone suggested I tighten the cable by turning the knob counterclockwise in back.
I tried this and maybe I turned it the wrong way but it seemed to loosen it. My shifting got much worse. I tried going the other way and all combinations in between and nothing works.
It skips gears, never clicks in properly and it driving my crazy. Questions:
Is a rear derrailer with 6000 miles worn out? Is there something I can do differently as far as adjusting it? I have my original chain also but I don't think that can be causing the horrible shifting I have now.
Assuming it's not worn out, is this something I can realistically fix myself or should I go to my LBS?

|I'm sure the derailleur is fine. Your probably in need of a new chain and Cassette. Other than that, you could try making sure everything is clean and lubed, ever check under the botton bracket to see if the cables are all mucked up. Shift to your small cog, loosen the pinch bolt on the der., grab it with a pair pliers , pull tight and lock the cable down. If anything, you will have to back out the adjuster nut on the der. 1/2 to 3/4 of a turn.
