|I don't know what's wrong, but my helmet stinks like nothing else. It isn't old (around 3 years), and I hadn't used it much until this year. Even still, I only ride 2-3 times a week, and no more than 1.5 hours each time.

|Those pads in the helmet need to be cleaned. You can remove the pads and throw them in the wash or just dump the helmet in the sink with some soap and clean it.
I know someone who wears his helmet in the shower. Soaps his head, puts helmet on, and wiggles it. Rinse, repeat.

|Remove the pads; I hand wash mine when I do gloves. Spray down the styrofoam shell of the helmet.
Otherwise, replace it. I had a Bell which I eventually tossed because it was uber-funky.

|Yup, remove the pads and wash with your other stuff (maybe in a delicate garmet bag?)
Since the straps stink, too, and they don't come out easily, I submerge the whole helmet in a bucket with an anti-bacterial soap mixture and soak it.
