I have leaned by front wheel against my bumper while putting the bike on the roof rack... then hopped in the car and drove over my wheel, trashing the rim.

D'OH!

Even worse, I couldn't find the exct same rim to replace it, so for the next year or so I had two different rims and just dreaded being asked why.

diving down the road and seeing your front wheel launch off the top of your car at 80 miles an hour because you forgot to tighten the quick release to the wheel holder on the roof rack.

D'OH

I tried mouth to mouth but the rim and spokes had to be amputated.

This is exactly why, when I bought my new car, I made sure it had a tow hitch. I knew that I had to get a tow hitch-mounted bike rack or I would do exactly that.
How about the dreaded quad..Spoke Wrench

whereby you damage your bicycle, your bike rack, your car, and your garage door all in one thoughtless moment.

I've never gotten the garage door, but I did get a canopy outside of a restaurant once. The canopy height was about 8'. My car with the tandem on the roof tops out at about 8'1" at the back of the stoker's saddle. The bike was OK, but damage to the car was about $500.00.

Drove my old C'dale MTB to the car wash one day. Clipped the handlebars on the top of the wash bay....old bike, no damage. I was washing it to give to my old man as a gift anyway.

A couple of weeks ago a friend of mine snagged an overhead cable in a driveway with his seat. Net result - he now has a setback seatpost, Yakima cross bars are toast and they don't get cable TV anymore. Frame survived.

and destroyed my rear wheel on a car wash header. It was a very costly screw up.
