Bob Roll suggested that the rider might be responsible for cause this failure. Is that a stress or fatigue thing? Or did the mecanic screw up which seems more likely?

Wouldn't it be partly the rider's job to check the bike after warming up, kind of like a pilot's safety check of a plane? It was the first time I saw anything like this & I guess it doen't happen often.

Mostly likely scanario was that one of the bolts on the bars or stem sheared off. It can happen under load. If the bolts weren't torqued properly, this would be far more likely to happen. That's why a torque wrench can be so important.
