Road shoes for the hard-to-fit?alex the engineer

I'm in the process of looking for a new shoe, but want to find one that actually FITS me this time. I have a high arch and a narrow heel. If a shoe fits my heel, then the toes are crammed in too tight. A roomy toebox means that I need heel inserts.
I would prefer laced shoes, but aside frome some funny-looking Italian racing shoes with more holes than swiss chese, I can't find any! The selection of shoes at my LBS's are pathetic, even at the roadie shop. If I order by mail, I'm going to probably go through 4 or 5 pairs before I find one that fits adequately, and that's a lot of return postage to pay for!
Any suggestions??
