The first one I bought was a Cinelli Grammo, which was measured along the side, from the center of the steering tube to the center of the bars. These stems are almost 1cm longer than other brands of the "same" size. Don't know if all Cinelli threadless stems are measured like this.

The more conventional method is measuring fom the center of the top cap bolt to the center of the bars, along the top of the stem. Very easy with a tape measure or ruler.

The 'more conventional' method applies primarily to the old quill type cast or forged aluminum stems. The Cinelli example applies to generally all thredless types as well as most recent tig welded cromo and aluminum quill types, with the measuerment taken from the center of the quill or steertr tube clamp, along to extension to the center of the bar clamp.

I measured my 110mm ITM Millenium stem, and it measures about 116mm along the top of the stem, which would make it 110mm at the center of the bars.
