Someone on Ebay is selling this funky fork/aerobar like what Chris Boardman used to ride in time trials. Is a setup like this custom or can you buy them pre-made? If its custom, where would you have one made?

Looks like a cowhorn with a connector...Cory

You can buy the basic cowhorn bar several places, including Nashbar (either there or Performance--I saw it in somebody's catalog recently). Or you can just flop a drop bar over and cut it off and come close to that same shape. I used to have one of those on my beater, because I never used the drops anyway.
The piece that goes between the bar ends (assuming that's what it is; the picture's pretty hard to decipher on my screen) used to be available, too, but I haven't seen one in years. If I really wanted one, I think I'd take the bike to somebody who can bend tubing and show him or her the picture. It shouldn't be hard to make something that would slip just inside (or outside) the bar, then secure it with tape or a rivet.
I believe Hotta made Boardmans (nm)J.S.

Lotus made his bikes for a couple years, but the frames were not very reliable so he started getting them from Hotta. Hotta still makes the frame but I don't think they ever made the fork/bar combo that he used.
I thought I read somewhere.....J.S.

That when Boardman was forced(Look made him) to ride the Look TT frame that he couldn't get low enough with their setup so he had Hotta make him that special fork bar setup like he was running on his previous bike(lotus-Hotta).
