I have seen some custom bikes for kids with a 19-25 inch inseam. Anyone have experience building up this size bike?

When they are built to the size you need, 20" BMX 1-1/8 rims are used on standard hubs up front and with a freewheel in back. I've seen STI and friction used. Make use of road and BMX equipment. For example, BMX dealers sell shoes small enough and clipless systems are availiable. Vittoria even makes sew ups for 20' wheels! Good luck.
