I have a Gunnar Crosshairs, used for commuting, which has 2 chainrings and a 12-28 cassette. Am looking for a 12-34 cassette to help on hills. Where can I find one? Will I have to modify rear derailleur or chain to use it?

Shimano and SRAM both make 12-34 cassettes, they should be pretty widely available. You don't say what rear derailer you are using. You may need to go to with an mtb derailer, depending on the capacity of your current one. Also, you'll likely need a slightly longer chain. It's a good idea to throw on a new chain whenever you put on a new cassette.
