I have a dura ace drive train. When I'm in my 4th lowest gear the chain slips or skips to the next highest gear.Is this an adjustment problem or is something wrong with the cog.Everything works great except in this one gear.

|If it was an adjustment issue, I'd think it would show up in some other gear combination too.
Cog wear, however, can be selective. The cogs that you use the most get the most wear. Then, when you get a new chain, their the most sensitive to the new chain/old cassette mismatch.
If that one cog is the only one with the problem, it's possible to buy just that cog and the adjacent one that it's riveted to.
