I don't know how well known this is, but I just had a problem with a rebuilt wheel where the LBS used Pedro's rim tape, which slipped due to poor adhesive. They have since gotten rid of it and use velo. Just an FYI.
Probably just wasn't wide enough.Spoke Wrench

Rim tape should completely fill in the space between the rim flanges. If it's a millimeter or so too narrow, it sometimes migrates to one side and exposes a spoke hole. None of the manufacturers makes all of the sizes necessary to fit every rim perfectly.

And the LBS said they had had a lot of similar problems...may have been just a bad batch from Pedros.

Salsa red rim tape is also junk. It too has a tendency to move around and expose holes. I havw seen it bunch up in the middle of the rim channel, becoming half as wide. Defies logic, seems that the air pressure would prohibit movement. Velox is for me now.
