Do I measure from the center of the head tube to the center of the handlebar? From the rear of the stem to the end of the stem? Something else?

You measure center to center, but usually not along the top of the stem, but along an invisible line. ie. The shortest distance between the center of the handlebar and the center of the steerer. The measurement is most always an even number like 90, 100, 110, 120, 130mm.
